Your insurance benefits will begin the first of the month following date of hire.  Please discuss PTO with your manager.
 
Health Insurance:
There are two available SelectHealth options; the SelectHealth care+ and the SelectHealth healthsave plan.

Dental Insurance:
There are two plans available through Total Dental Administrators (TDA).  For details regarding these two plans, please see the TDA booklet.
 
Vision Insurance:
There are three plans available: Excel EyeOpticare of Utah, and VSP.  There are also discount offerings available through Opticare of Utah via your TDA dental insurance at no additional cost.

Other:
Flexible Spending is available through PayFlex.  If you desire to have a Health Savings Account (HSA), you must choose the SelectHealth health save plan (see the HSA Information section below for more details regarding an HSA).
