COP/CPB-ESG Benefit Summary

Your insurance benefits will begin the first of the month following date of hire.  Please discuss PTO with your manager.

Health Insurance:
SelectHealth is the carrier for the group health insurance.  Click here to view the plan details.

Dental Insurance:
There are two plans available through Total Dental Administrators (TDA).  For details regarding these two plans, please see the TDA booklet.
 
Vision Insurance:
There are three plans available: Excel EyeOpticare of Utah, and VSP.  There are also discount offerings available through Opticare of Utah via your TDA dental insurance at no additional cost.

Other:
Flexible Spending is available through PayFlex.
